DISEASE MANAGEMENT
DEMAND MANAGEMENT
|
Disease Management consists of medical, patient-education
and case management for diabetes, asthma, and other diseases. The goal is to assist
both doctor and patient to do everything needed to slow progression of the disease and
prevent medical complications.
Demand Management is a more general term. In
addition to disease management, it includes immunization, health education, and other "primary"
preventive services. It includes pap smears, routine physical and other "secondary"
preventive services for early detection of serious illness. Demand Management also
includes non-preventive policies and procedures to reduce healthcare costs. These
can include nurse-answer lines to reduce emergency room visits, co-payment schemes to
encourage use of less costly medications and less-costly healthcare services, and other
interventions.
JLN, MD Associates can
provide Disease Management and Demand Management Consulting services to:
 | Determine the most cost-effective mix of
Disease/Demand Management (DDM) services for an insurer, provider or community. |
 | Design, plan and help implement DDM services for
insurers and providers who would rather "build" or "assemble" their
own programs as opposed to "buying" them from a DDM vendor. |
 | Design, plan and help implement both clinical and
community preventive services |
 | Establish baselines, benchmarks and goals for DDM
programming |
 | Assist with related elements of HEDIS, HCFA and JCAHO
compliance |
 | Develop concurrent evaluation protocols for DDM
programming so the agency, insurer or provider can track process and outcomes from day 1
of implementation |
 | Trouble-shoot current DDM programming that seems too
costly, ineffective, or is not acceptable to clinicians, patients or the community
